Summary: | Aim– To assess the correlation between blood parameters and grade of hypertensive retinopathy in patients with Pregnancy Induced Hypertension (PIH) & to establish this relation as a predictor of hypertensive retinopathy in PIH.Study design– Prospective, comparative, case control studyMaterials and methods– For the purpose of this study 100 patients WITH PIH fitting under the inclusion criteria were taken. 50 cases of PIH with normal fundus on direct ophthalmoscopy were considered as controls. 50 cases of PIH with retinopathy changes in fundus on direct ophthalmoscopy were considered as cases. Initial routine eye examinations – from 28 weeks – 32 weeks of gestation or at the time of diagnosis of PIH (whichever is earlier). Second routine eye examination performed at term (40weeks) / at the time of active labour (whichever is earlier).Results– In Retinopathy group, 52% had APE, 26% had SPE, 10% had NSPE, 8% had IE, 2% had HELLP and IPE respectively. In Non retinopathy group, 50% had NSPE, 40% had SPE and 10% had IE. There was significant difference in diagnosis between two groups. (p<0.001). There was significant difference in mean Uric acid, ALT, AST between two groups.(p<0.001) Uric acid, ALT and AST was higher in Retinopathy group than in non retinopathy group. Uric acid >6mg/dL had sensitivity of 66%, specificity of 100%, PPV of 100% and NPV was 74.6%.Conclusion– Hypertensive retinopathy in PIH is a sign of end organ damage. Uric acid levels are sensitive predictors of hypertensive retinopathy in patients with PIH. Correlating the severity of hypertensive retinopathy and levels of uric acid suggests a common pathophysiological cascade. |
