Summary:Radiotherapy is one modality of radical therapy for patients with localized prostate cancer without severe complications. Traditionally, it has been given with a conventionally fractionated schedule over several weeks; however, moderate hypofractionation has emerged as a standard of care over the past few years, based on data from large randomized studies.^1-4^ Another important topic in this clinical setting is elective irradiation of the pelvic lymphatics but clinical data and guideline recommendations remain uncertain.^5-8^ Radiotherapy is also associated with a different spectrum of side effects than surgery or active monitoring,^9,10^ which can, alongside efficacy outcomes, have a substantial impact on decision-making. This article summarizes data on these important issues in prostate cancer management, including recent updates. PEER REVIEWED ARTICLE
