Summary:efixime (CFIX), a newly developed oral cephalosporin which is stable to various kinds of β-lactamase and has broad antibacterial spectrum, was evaluated for efficacy and safety in 23 otorhinolaryngological patients. And the penetration of the drug to faucial tonsil, mucosa of maxillary sinus and otorrhea in human and middle ear mucosa of guinea pig were experimented. 100mg of CFIX was administered twice daily to one case of acute otitis media, 4 of acute exacerbation of chronic otitis media, 3 of acute sinusitis, 7 of acute exacerbation of chronic sinusitis, 5 of acute tonsillitis and 3 of acute pharyngitis. Clinical efficacy of the drug was excellent in 15 cases, good in 7 cases and poor in one case. The efficacy rate of the drug was 95.7%. In bacteriological effect, all the bacteria but only one strain of P. aeruginosa isolated from poor case were eradicated. There was no side effect and no abnormal laboratory test value. In experimental study, tonsil and maxillary sinus mucosa was collected from the patients who were operated tonsillectomy or sinectomy. CFIX was administered about 3-4 hours before surgery. Otorrhea was collected with time after dosing of 100mg of CFIX. On the other hand, 200mg/kg of CFIX was administered intraorally to guinea pigs which had been occured otitis media by inoculating 0.1ml of 107CFU/ml of S. pneumoniae in the middle ear the previous day. The middle ear mucosa was collected by surgery with time. Concentration of CFIX was assayed by disc method using E. coil ATCC 39188 as test organism. Concentration of CFIX in faucial tonsil and maxillary sinus mucosa were about 21-48% and 23-47% of serum concentration, respectively. And the drug was detected in otorrhea with the level of 0.16pcg/ml 6 hours after dosing. In the experiment of guinea pigs, concentration of CFIX in middle ear mucosa were about 25-43% of the serum concentration until 6 hours after administration.
