Highlights • Intracranial arterial dissections presenting with convexity subarachnoid hemorrhage are rare • Even if the dissection is suspected, misdiagnosis is not infrequent • Thrombus within the dissected segment may even reveal fake images of saccular aneurysms • This report illustrates likely pitfalls in the diagnosis and management of intracranial dissections
