Summary:1. 1. The apparent rarity of fully proved instances of sinus premature systoles is discussed and criteria for their diagnosis are reviewed. 2. 2. A third case of sinus premature systoles satisfying these criteria is presented and added to the two already in the literature. 3. 3. Two additional cases are described in which a careful analysis of the tracings warrants their consideration as examples of sinus premature systoles. 4. 4. It is believed that the rarity of this arrhythmia is more apparent than real and that careful examination of some bigeminal rhythms ascribed to alternate atrial premature systoles might reveal these premature systoles to arise in the sinus node.
