Summary:The alteration of the gastrointestinal anatomy represents a challenge for the performance of endoscopic procedures in case of bile duct obstruction. Follow we present the technique used in a patient with altered anatomy and a mass in the head of the pancreas with obstruction of the bile duct through endoscopic ultrasound-guided trans gastric retrograde cholangiopancreatography.
