Summary:The difference between the entry requirements to specialist training-the specialist registrar (SpR) grade-and to general practice vocational training schemes is striking. Basic language ability and communication skills should be assessed at selection by means of a first part royal college examination using written papers and video analysis of consultations with simulated patients, techniques which are now well validated.
