Summary:Antibacterial activity against urinary bacteria, pharmacokinetics and clinical efficacy of KW-1070, a new aminoglycoside, were studied and following results were obtained. 1) MICs of KW-1070 were almost equal to those of gentamicin for S. marcescens but 2 to 4 times higher than those of gentamicin for P. aeruginosa. 2) A peak serum level of 14.7 μg/ml was achieved one hour after i. m. administration of KW-1070. In patients with reduced renal function, prolonged serum half-lives and lower urinary recovery rates were observed with decrease of renal function. 3) Tissue concentration of KW-1070 in kidneys with markedly reduced or almost no function were higher than expected. Concentrations in urinary bladder wall and prostate were higher than MICs of KW-1070 for most species of bacteria. 4) Effectiveness rate of KW-1070 for 17 patients with chronic complicated UTI was 41%. Dose response was not observed between cases treated with i. m. administration 200mg and 400mg of KW-1070. 5) Slight increase of GPT and/or GOT was observed in 2 out of 20 cases treated with KW-1070 in this study. Other laboratory findings showed no abnormality.
