Summary:Sarcoidosis is a systemic inflammatory disease characterized by noncaseating granulomas of unknown origin. Erythema nodosum is the most common nonspecific cutaneous lesion of sarcoidosis. In most cases it occurs with subcutaneous erythematous nodules, often observed on the anterior tibia, accompanied by systemic symptoms, such as fever, malaise, or polyarthralgias. Clinical manifestation includes chronic and indurated papules or plaques in the middle face, particularly the alar rim of the nose. A 49-year-old woman presented with hardened nodules in the malar region, initially unilaterally and later bilaterally, with painful symptoms, having been submitted to incisional biopsy. Ultrasound, computed tomography, and magnetic resonance were performed, and the findings were unspecific for both the nodules of the face and those in the lung. The patient was referred to several medical specialties and the diagnosis of exclusion was sarcoidosis. Treatment involves use of corticosteroids and aims to control the disease.
