In this article, we present the case of a 46‐year‐old transgender woman who underwent gender‐affirming vocal surgery. We will discuss the modified Wendler glottoplasty surgical technique, anaesthetic management and postoperative recovery. We also discuss future peri‐operative implications, including airway management for those undergoing general anaesthesia for subsequent operations.
