Summary:[...]pineal region metastases are very rare; according to recent statistics, they account for only 0.3 % of all intracranial metastases [3]. [...]the pineal region is a site of predilection for primary tumors of the pineal region and germ cell tumors. [...]when lesions are seen in the pineal region, these tumors have higher priority in the differential diagnosis. [...]this very rare case is reported. [...]beginning in early July 2012, about 1 year after treatment, the patient had a gradual decline in activity levels and developed impaired consciousness. Since imaging studies showed a tumor in the pineal region and cerebral ventricular enlargement, she was referred to our department. [...]of hospital admission, the patient’s level of consciousness was JCS 3 (GCS 14), and she was disoriented and had an ocular motility disorder. MRI showed a decrease in the ventricular enlargement. Since intraoperative findings showed tumor dissemination in the cerebral ventricle, whole-brain irradiation (37.5 Gy) and booster irradiation (7.5 Gy) to the pineal region were given.
