Summary:•VATS – Wedge Resection (WR) is a minimally invasive video-assisted, non-anatomic limited resection of a lung portion.•Erector Spinae Plane Block (ESPB) may be a new option for chest wall and upper abdominal analgesia.•The patient was at high risk for general anesthesia and, given her therapy with clopidogrel, for neuraxial or paravertebral anesthesia too.•We performed ESPB and mild sedation, leaving the patient spontaneously breathing without the need of any supraglottic device.•ESPB granted a good perioperative performance, pain-free postoperative period and our patient’s total satisfaction•ESPB could be an alternative to General Anesthesia or Serratus Anterior Plane Block.
