Summary:•SLNB is comparable to END in terms of Overall Survival in patients with early stage OSCC.•SLNB can provide a significant reduction in post-operative morbidity particularly in OSCC involving subsites such as the floor of the mouth.•99mTc-tilmanocept leads to identification rate for the floor of the mouth subsite of 100%, with a 0% false-negative rate and high negative predictive value, reducing “shine-through” phenomenon.•SLNB in early stage OSCC patients is a reliable technique, to be performed preferentially in a multidisciplinary setting in trained centers
