Background In 2014, an antimicrobial stewardship programme (AMSP) for carbapenems was implemented in our hospital. Based on the good results of this approach, we decided to expand the AMSP to review all antimicrobial treatments that lasted for more than 21 days. Purpose To describe, analyse and evaluate the AMSP recommendations, and the acceptance rate of long duration antimicrobial treatments. Material and methods In this prospective study, lasting 4 months (June-September 2106), we included all admitted patients to a general university hospital receiving treatment with the same antibiotic agent for at least 21 days. A multidisciplinary team of antimicrobials experts evaluated each patient's treatment every day and made a recommendation to the responsible physician. The studied variables were: prescribed department, antibiotic, indication for treatment, recommendation and if this recommendation was accepted or not. Results We included 54 patients (61 antibiotic treatments). In 67.2% of cases, the recommendation was to finish the treatment, in 24.6% to continue in the same way, in 3.3% to de-escalate the treatment, in 1.6% to change the dose and in 3.3% another type of recommendation was made. 54% of treatments were empiric. 76.3% of recommendations were accepted by the responsible physician. The most prescribed antibiotics were third generation cephalosporins (18%), piperacillin-tazobactam (14.7%), quinolones (9.8%) and amoxicillin-clavulanic (9.8%). The medical department prescribed 70.5% of antibiotic treatments and acceptance was 79.1%, the surgical department prescribed 27.9% of treatments (acceptance 58.2%) and the intensive care unit prescribed 1.6% of treatment (100% of recommendations were accepted). Conclusion We found that 3/4 patients on antibiotic treatment for at least 21 days have inadequate treatment. In addition, most treatments were empiric and used broad spectrum antibiotics. This suggests that this approach, that reduces the duration of antibiotic treatments, also could reduce the incidence of multiresistant bacterias. The medical department acceptance rate was better than the surgical department, and hence additional initiatives to improve the acceptance rate may be necessary (educational or training actions or protocol reviews).
