Summary:The aim of this grounded theory study was to discover the basic social and psychological processes and problems of nurses that explain interaction in a new Patient Focused Care delivery system. These basic social and psychological processes became the organizing framework for the explanatory substantive theory of empowered self-management. Consistent with a grounded theory approach, the basic social/psychological processes and basic social problems were derived from the social world of nurses implementing the new delivery model, Patient Focused Care. The focus of the study became exploring the path and process of empowerment which led to the discovery of self-management. The related problematic area became the exploration of the nurses' lack of knowledge/experience in the areas of: advanced clinical decision-making, work allocation, communication and management. The basic social/psychological processes and problems of nurses were derived from interviews, participant-observation, and pertinent documents. Data were collected over a five month period. Using a constant comparison approach, data collection sampling strategies were modified as the processes and problems were identified and explored. As the study evolved, sampling became more focused on those nurses who demonstrated the drive toward empowerment and self-management. This research study presents the use of a grounded theory approach in discovering the substantive theory of empowered self-management. Recommendations for nursing administration, nursing education, and nursing research are presented.
