Summary: | Hepatitis B virus (HBV) is the most efficiently transmissibleof the bloodborne viruses that are importantin healthcare settings. Healthcare workers (HCWs)are at risk for exposure to HBV from infected patientsand, if infected, are similarly at risk of transmittingHBV to patients. Published cases of HBV transmissionfrom HCW to patient are relatively rare, having decreasedin frequency following the introduction ofstandard (universal) precautions, adoption of enhancedpercutaneous injury precautions such as double-gloving insurgery, and routine HBV vaccination of HCWs. Here wereview published cases of HCW-to-patient transmissionof HBV, details of which have helped to guide thecreation of formal guidelines for the management ofHBV-infected HCWs. We also compare the publishedguidelines for the management of HBV-infected HCWsfrom various governing bodies, focusing on theirdifferences with regard to vaccination requirements, viralload limits, frequency of monitoring, and restrictionson practice. Importantly, while there are differencesamong the recommendations from governing bodies, noguidelines uniformly restrict HBV-infected HCWs fromperforming invasive or exposure-prone procedures. |
