Summary: | A 73-year-old man presented with a 4-month history of a rapidly growing exophytic mass in the right mandible. Imaging exams revealed a destructive radiolucency causing tooth resorption. An incisional biopsy was performed, and microscopic analysis revealed the proliferation of medium to large-sized anaplastic cells with abundant cytoplasm, eccentric nuclei, marked pleomorphism, and numerous mitoses. The neoplastic cells tested positive for CD138, VS138c, lambda, and MUM1, while testing negative for AE1/AE3, kappa, S100, synaptophysin, and EBV (in-situ hybridization). The Ki-67 labeling index was 70%. The patient's systemic clinical checkup ruled out other bone lesions, confirming the diagnosis of an anaplastic plasmacytoma (AP) of the mandible. Three weeks later, the patient returned with breathing difficulties due to a large increase in the lesion. The patient is currently undergoing oncological treatment. Although AP is a rare neoplasm, it should be considered in the differential diagnosis of anaplastic cell proliferations in the oral cavity of elderly patients. |
